This is Billy Jericho.



I have returned from my journey. I paddled my way around the world in a canoe. It did not have wifi, so I haven't posted on this board for over ten years. I want you to imagine paddling a canoe around the world for ten years, and then imagine how happy I am to be back amongst those on this board. My loved ones.



For the last ten years I have subsisted on tuna blood for hydration and pistachios for protein and calories. I went through a great deal of trouble to catch the tuna, and I now wonder why I didn't eat the tuna but instead simply drank tuna blood.



I have seen many places and done many things. I will be sharing these with you, my followers, over the next few weeks. For now, please know that I have returned and I've survived, thrived and revived. I have decided to start my own religion that will be dedicated to the greater power and glory of me. I know I can count on each of you to come to church on Thursdays to pray to me and my little brother, Jerixxo.

"Team Onyx who ever you are. The reason TH is being so nice to you and why you have preferential treatment is because TH pushed hard for your acquisition and quite frankly overpaid for Onyx. You aren't a bad buy, but still haven't proven to be worth the $10B since that was based off a forecast of generating at least $2B a year. TH knows that he will look bad if you can't deliver so he is willing to give you far more resources in terms of budget and headcount for you to make those revenue numbers. Noticed I said revenue. While other legacy Amgen business units will be judge of profitability, you get the pass on that as long as you hit top line numbers for now. Much like how VB has stripped other BUs of resources for CV, you get the same benefit. This is why the OBU and INBU are being downsized because both CV and Onyx are considered growth products."

ummmm....Where the nearest exit ?



Living the dream, late 20's making 6 figures..company car, good benefits, free gas but loosing my mind.



Where do I begin?...been in Pharma for a little over a year and it finally hit me I'm not cut it out for this travesty. My gosh...its hard to up get it in the morning. Yes i've had others jobs were the work was more and pay much less and customers where tough to deal with but the mind f*ck of this industry is unparalleled.



There is absolutely no respect or value in this field. I joined pharma thinking I was going to have an impact on people, a rewarding job where I was respected by my customers and peers. A place where advancement opportunities came to those who put in the time and worked hard. A company where my supervisor didn't play favorites and invested his time in my development, a place where people worked as a team and collaborated to increase sales.....but boy has this been anything farther from the truth. Instead my job description best suits a clown with his red nose and floppy shoes who works in a circus and sole purpose is to entertain the office cows in return of access and scripts.



In this job all laws of logic, science and reason are thrown out the window and if you dare use them you are the bad guy! If you want to get into pharma consider the following..



#1 Laziness has killed this industry. The lazy rep becomes the lazy manager, the lazy manager expect knowledge and results of things he was too lazy to address. Your lazy counterparts are offended if you log in too many calls or work a full shift. Oh, and heaven forbid you get to see a doctor more often by waiting a bit longer than they are used to, then your added effort will be seen as call falsification and will need to be explained with your manger on your next field ride.



#2 Avoid jobs with Pods - Honey Booboo has been with the company...oh my... for 4 years and she has still not gotten her promotion!!! Well, Honey Booboo will and start canceling your lunches and appointments with doctors behind your back, accuse you of call falsification and report the slightest mishap just to make sure you don't get ahead in line (True story). Trust no one. Anything past hello or goodbye will be scrutinized and used against you in the court of morons.





#3 Do you need a signature?! - look around if you don't see anyone from Fedex or UPS then this is you cue to put on the show! start juggling chainsaws as you deliver your 10 second detail approved by your retard manager. Don't worry about the mindless look on the Doctors face after all you dropped the samples and gave them lunch so your job here is done!



#4 The rejects ruin the culture - See that miserable rep from Pfizer or Merck on other blog hating his life and the values of his company...well guess what?! soon he will be your new manager...Surprise!! Ironically he'll start implementing the same stupid tactics down the food chain and pass on the misery from all his previous jobs on to you.



#5 Check your ego at the door...no better yet throw it out the window. The longest intellectual conversation you will have with anyone in an office will be with the MA about last night's episode of the Kardashians or about what type of food you are bringing next time. Would you like fries with that? White or wheat? Diet or regular? Somedays I swear i'm fooled into thinking that I work for either UPS or McD's or both! (Disclamer: nothing wrong with working at McDs or UPS both of these professions have more value than a pharma rep)



#6 - The higher your emotional intelligence the more you will suffer. If you held any real sales job before AND were ever good at it then you've developed the ability of sensing emotions and picking up cues rather quickly. The moment you realize that on your best day the same value you bring can be done by a 12yr old delivery boy then thats the day the floor will be removed from under you. Hence, the industry is left with either A) individuals with no emotional intelligence AKA retards, meatheads and bimbos or B) those who have sold out for the money...same class as strippers, prostitutes, used car salesman and debt collectors.



#7 Being a pharma rep is like buying a new car, the more miles you have in the industry the less value you in the market. "Pharma Reps need not apply" or "Will entertain the idea of pharma reps with no more than 5 years of pharma experience" are the words under some of the other sales jobs I've encountered. Your better off deleting pharma of your resume and just telling your new employer that you just woke up from a 5 yr self induced coma.

Last week, Teva Pharmaceutical Industries Ltd. (TEVA) announced that an additional administration option for Granix injection has been approved by the FDA. The FDA has approved Granix for self-administration by patients and caregivers.



Teva plans to launch a new Granix syringe for self-administration in early 2015. The currently available Granix syringe is indicated only for administration by a health care professional.



Granix is approved for the reduction in the duration of severe neutropenia in patients with nonmyeloid malignancies receiving myelosuppressive anticancer drugs associated with a clinically significant incidence of febrile neutropenia.



We are pleased with the FDA approving Granix injection for self-administration by patients and caregivers. It will provide a convenient option for patients who have an issue with visiting physicians multiple times.



Other approved neutropenia drugs include Amgen Inc.'s (

) Neupogen indicated for the reduction of the risk of infection in patients with tumors, who are receiving strong chemotherapy that may cause severe neutropenia with fever.

I was recently told about a former Solstas Sales Director hired in Florida as a Director of Strategic Alliance who left Quest after about 6 months to work as a regional sales director at PCLS. Since she has been there she has solicited employees and hired at least one. I heard she was even given a teary good bye by the VP down there in Tampa? What? Florida is sure filled with crazies but the no state income tax, no non-compete enforcement and sunny weather have motivated me to look for work down there. The VP of Commercial seems to be a cream puff so this decision is looking even better.

In September the company presented the final results of a Phase III trial of Afinitor (everolimus) in patients with well-differentiated advanced and progressive pancreatic neuroendocrine tumors (pNET). This was part of the RADIANT-3 (RAD001 In Advanced Neuroendocrine Tumors) trial. mTOR (mammalian target of rapamycin) inhibition was confirmed in advanced pNET. The results of the study indicated a median overall survival (OS) of 44.02 months in patients receiving everolimus, compared to 37.68 months in those receiving placebo.

I'm currently a junior in a biomedical engineering program and just finished an internship at a top-tier device company. After getting experience as an engineer, I'm not sure if that kind of highly structured lifestyle is for me. I also really enjoy public speaking and giving presentations. I'm getting a business minor and am trying to get a feel for the medical device sales space. (Reuters) – The U.S. Food and Drug Administration approved a drug to treat dementia in Alzheimer’s patients that was developed by Actavis Plc and Adamas Pharmaceuticals Inc.



Adamas shares were up nearly 17 percent at $17.10 in premarket trading.



The drug, Namzaric is designed to treat moderate-to-severe dementia in Alzheimer’s patients by combining in a single capsule memantine and donepezil — ingredients in two drugs that are often prescribed together.



Memantine is the active ingredient in Actavis’ Namenda, while donepezil is the active ingredient in Pfizer Inc’s Aricept. Both are in use for Alzheimer’s-related dementia.



Dublin, Ireland-based Actavis said it expects to launch Namzaric in two dosage strengths in the United States in the second quarter of 2015.



Adamas will retain commercialization rights outside the United States.



Alzheimer’s is a progressive, neurodegenerative disorder characterized by problems with memory, thinking and behavior.

Fortunately, I've been placed with the farx/xigd team, but I am really concerned about my counterparts on the injectable team. I think they are the looser's in this reorg. How do you go down to one rep per territory and/or two reps splitting two full previous territories and expect to compete? They will be so outnumbered and resourced. It seems like they are giving up on Bydureon with Trulicity coming out. I would love to have my full team selling both orals and injectables. I think we could continue to grow, but these new injectable reps with be outnumbered anywhere from one to four or five reps per territory. SSLT--Thanks for listening and hearing us. As Auviq winds down a slow death in 2014 which made 2013 seem like a decade ago,the brand team along with leadership shit the bed in 2014. Quotas that were impossible, managed care an ongoing disaster made selling this talking turd an adventure. How could a company screw up this product any worse. A launch brand team that wasn't ready or prepared for managed care and then pricing it with no advantage. The product doubled in price in two years and the finger pointing by management and the brand team clearly showing how incompetent and out of touch our leadership was. The meeting in New Orleans was the first hint of trouble ahead. A clumsy thrown together meeting lacking content or leadership. One of the worst and that's says a great deal to the many worthless Sanofi meetings over the years.For us who are stuck with this talking turd, good luck as we'll need it with the quotas coming in 2015. This little company Mylan beat Sanofi like a drum and totally out marketed the brand team and were 5 steps ahead during the entire last two years. Look for Mylan again to be fully prepared Jan 1st 2015 to squash Auviq again. The one thing in our favor is that arrogant idiot Brian is out of the company. Completely full of it and couldn't pick a jackrabbit out of a herd of turtles. He should be compared to the pilot of the Valdez, Titanic, Greek cruise ship. Merry Christmas and make sure to decorate the tree with trainers. Look for Sanofi to get out of Auviq in 2015 pending a miracle. All is well with Funk at the helm. Finger pointing to start Jan 7 th after first week if numbers come in.

Biosimilars are a bittersweet affair for Amgen ($AMGN). The biotech has a biologic blockbuster that's sliding off patent next year, but it's also positioning itself to be a major biosimilar provider. No biosimilar applications have yet been approved by the FDA, although the agency released draft guidance on the clinical and regulatory pathway for biosimilars in May. The agency received comments on it into August.



Neulasta is a pegylated, longer-acting version of Amgen's Neupogen. Teva ($TEVA) already has EU-approved biosimilar versions of Neupogen and Neulasta. Other companies have also committed to making generic versions of Neupogen or Neulasta or both, such as Sandoz and Coherus. But Amgen itself hasn't said it will go after the biosimilar market for these products. Amgen does have its own full biosimilar pipeline from which it expects to start launching products in 2017.



In 2013, Neulasta had $4.4 billion in global revenues, making it one of Amgen's top revenue-generating products, just a bit behind Enbrel. In combination, Neulasta and Neupogen produced $5.8 billion in sales last year.



Amgen, along with AbbVie ($ABBV) and Roche ($RHHBY), are seen by Wall Street as most vulnerable to the nascent onslaught of biosimilars.

What makes this latest failure particularly noteworthy is that it added to the growing skepticism in the industry about the future of cancer vaccines in general. Despite an intensive research effort, cancer vaccines have repeatedly flopped in clinical testing, or at best, have only shown marginal benefits, when compared to other available treatments -- such as Dendreon's Provenge for prostate cancer, and Amgen's (NASDAQ: AMGN ) T-Vec indicated for metastatic melanoma.



Going forward, I expect big pharma to continue to move away from cancer vaccines to focus on other types of immune-based therapies like checkpoint inhibitors. Unless something drastically changes, cancer vaccines no longer appear to be strong clinical candidates as monotherapies, and might only serve a minor role as "immune enhancers," when used in conjunction with other types of treatments.

Why Merck going outside company to fill the IDS Lead positions?

Merck's has posted a number of "Integrated Delivery Network (IDN) Lead" positions.

Two questions:

(1) Why are we not promoting from within for these jobs? (2) Our "customers" would be members of the so-called "C-Suite." How and why are we trying to get in front of the "C-suite" customers?



I have talked to a number of friends outside Merck who have jobs like this and they all tell me that it is a joke. No CEO, CFO, CNO wants to hear from us. Most pharmaceutical companies respond with "we want 1 person as the point of contact" for an IDN/hospital/hospital system. What I have learned from most pharma folks (at least from those competitors I have spoken with) is that the person doing this job ends up trying to come to all the hospital reps appointments while never actually getting into the c-suite. Each company tries to sell their own version of a "value-proposition" as being so unique. However, customers see right through that. Thoughts everyone??

Alzheimer’s is thought to be caused, at least in part, by the buildup of β-amyloid plaques in the brain. Gantenerumab was designed to be administered early in the disease’s progression, before severe symptoms develop. Despite promising early results, the antibodies did not halt the formation of the plaques in any meaningful way, sparking new debate over how to treat the debilitating neurological disorder and whether the plaques are even its primary cause.

In Leaders Eat Last: Why Some Teams Pull Together and Others Don’t, author Simon Sinek suggests that the best organizations foster trust and cooperation because their leaders build what he calls a Circle of Safety. This separates the security inside the team from the challenges outside.



This Circle of Safety enables teams to be stable, adaptive and confident where every member feels they belong and are focused on the right things.



Part of the challenge in creating such a circle requires leaders at every level to maintain balance of four chemicals found in our bodies. These chemicals control our feelings, which are the primary drivers for all our decision-making whether we are aware of it or not.



Chemicals such as endorphins and dopamine function to get us where we need to go as individuals. Endorphins provide the “runner’s high,” which are able to mask pain and enable us to complete a marathon or complete a work project well into the night. Dopamine provides a feeling of satisfaction once we complete an important task on our to-do list. It provides incentive for progress toward reaching our goals.



These two are what Sinek refers to as the selfish chemicals and they provide us with short-term rewards, which can motivate us to accomplish great things and, under the right conditions, can also become addictive.



On the other hand, serotonin and oxytocin work to help strengthen our social bonds so we are more likely to work together and cooperate well. Serotonin and oxytocin are what Sinek describes as the selfless chemicals and they are sorely missing in leadership today.



Serotonin is responsible for the pride we feel when those we care for achieve great things. As the boss, serotonin works to encourage us to serve the employees we are responsible for. And as the employee, serotonin encourages us to work hard to make the boss proud.



Serotonin more than any of the others is seen as the leadership chemical.



Oxytocin is the chemical that helps us direct how vulnerable we can afford to make ourselves. This social compass helps determine when it’s safe to open up and trust or when we should hold back. This might be the drug most closely aligned with emotional intelligence.



Oxytocin makes us better problem solvers and enables us to accomplish more in groups than we can alone. It has also been found to contribute to us living longer.



The goal of any leader should be to find balance. If you remain addicted primarily to endorphins and dopamine, no matter how rich and powerful you become, you will likely feel lonely and unfulfilled. On the other hand, if you are focused too much on serotonin and oxytocin, you may lack the measurable goals or ambition necessary to reach important feelings of accomplishment.

Forcing us to pay for a company car is wrong

We should not be forced to pay a fee each month for a company vehicle if we only use the car for work related miles. We all do our administrative work from our home (reports, print, emails, etc). We technically have a home office and if the company denies that then our only job is to physically stand in physician offices. Perhaps we should ignore the other responsibilities including planning and data analysis. Eliminate email accounts because we can't read emails in front of doctors - right?

One way to avoid the harsh commuting rule is to have a home office that qualifies as your principal place of business. In this event, you can deduct the cost of any trips you make from your home office to another business location. For example, you can deduct the cost of driving from home to your outside office, a client’s office, or to attend a business-related seminar. The commuting rule doesn’t apply if you work at home because, with a home office, you never commute to work (you’re there already).



Your home office will qualify as your principal place of business
 if it is the place where you earn most of your income OR perform the administrative or management tasks for your practice. If your home office qualifies as your principal place of business, you can vastly increase your deductions for business trips.



Example: Kim (from the above example) maintains a home office where she does the administrative work for her business; she also has an outside office where she does her other work. She can deduct all her business trips from her home office, including the 20-mile daily trip to her outside office. Thanks to her home office, she can now deduct 100 miles per week as a business trip expense, all of which was a nondeductible commuting expense before she established her home office.

Health Consultation Services is new to the Iowa market and we are currently hiring a Part Time doctor in the Des Moines area.



We are looking for a doctor to provide a very simple service of signing off on test sheets for pharmacogenetic buccal swab test. We provide these tests to Medicare patients in Iowa through health screening expos that are directed at Medicare patients. Medicare pays for the pharmacogenetic test at 100% with no copay and our lab directly bills Medicare.



Our lab, Affiliated Genetics in Salt Lake City, Utah, has been a leading DNA testing lab for over 20 years.



Our current fee schedule is to provide our doctor with $10 per swab sign-off, up to $500 per week. We will renegotiate a larger fee as more tests are being performed each week. It takes doctors less than 1 minute to sign off on each test sheet.



Process: Our staff collects the buccal swabs at events, and patients provide a list of current medications, and we make a copy of their medicare card. We then enclose these items in an envelope for each patient and then all we need is the doctor to sign off on the test for each patient. Again, it takes doctors less than 1 minute to sign off on each test sheet.



We are HIPPA compliant and our doctors must have a valid individual National Provider Identifier (NPI) and be PECOS registered online.

Christmas with Louise - a Christmas story

As a joke, my brother used to hang a pair of pantyhose over his fireplace before Christmas. He said all he wanted was for Santa to fill them. What they say about Santa checking the list twice must be true because every Christmas morning, although Jay's kids' stockings overflowed, his poor pantyhose hung sadly empty.



One year I decided to make his dream come true. I put on sunglasses and went in search of an inflatable love doll. They don't sell those things at Wal-Mart, you know. I had to go to an adult bookstore downtown.



If you've never been in an X-rated store, don't go. You'll only confuse yourself. I was there an hour saying things like, "What does this do?" "You're kidding me!" "Who would buy that?"



Finally, I made it to the inflatable doll section. I wanted to buy a standard, uncomplicated doll that could also substitute as a passenger in my truck so I could use the car pool lane during rush hour. Finding what I wanted was difficult. "Love Dolls" come in many different models. The top of the line, according to the side of the box, could do things I'd only seen in a book on animal husbandry.



I settled for "Lovable Louise." She was at the bottom of the price scale. To call Louise a "doll" took a huge leap of imagination. On Christmas Eve and with the help of an old bicycle pump, Louise came to life. My sister-in-law was in on the plan and let me in during the wee morning hours. Long after Santa had come and gone, I filled the dangling pantyhose with Louise's pliant legs and bottom. I also ate some cookies and drank what remained of a glass of milk on a nearby tray. I went home, and giggled for a couple of hours.



The next morning, my brother called to say that Santa had been to his house and left a present that had made him VERY happy but had left the dog confused. She would bark, start to walk away, then come back and bark some more. We all agreed that Louise should remain in her pantyhose so the rest of the family could admire her when they came over for the traditional Christmas dinner.

My grandmother noticed Louise the moment she walked in the door. "What the hell is that?" she asked. My brother quickly explained, "It's a doll." "Who would play with something like that?" Granny snapped. I had several candidates in mind, but kept my mouth shut. "Where are her clothes?" Granny continued. "Boy, that turkey sure smells nice, Gran, " Jay said, to steer her into dining room. But Granny was relentless. "Why doesn't she have any teeth?" Again, I could have answered, but why would I? It was Christmas and no one wanted to ride in the back of the ambulance saying, "Hang on Granny, hang on!"



My grandfather, a delightful old man with poor eyesight, sidled up to me and said, "Hey, who's the naked gal by the fireplace?" I told him she was Jay's friend. A few minutes later I noticed Grandpa by the mantel, talking to Louise. Not just talking, but actually flirting. It was then that we realized this might be Grandpa's last Christmas at home.



The dinner went well. We made the usual small talk about who had died, who was dying, and who should be killed, when suddenly Louise made a noise like my father in the bathroom in the morning. Then she lurched from the panty hose, flew around the room twice, and fell in a heap in front of the sofa.

The cat screamed. I passed cranberry sauce through my nose, and Grandpa ran across the room, fell to his knees, and began administering mouth-to-mouth resuscitation. My brother fell back over his chair and wet his pants. Granny threw down her napkin, stomped out of the room, and sat in the car.

Later in my brother's garage, we conducted a thorough examination to decide the cause of Louise's collapse. We discovered that Louise had suffered from a hot ember to the back of her right thigh. Fortunately, thanks to a wonder drug called duct tape, we restored her to perfect health.



It was indeed a Christmas to treasure and remember.

G0416 Surgical pathology, gross and microscopic examination, for prostate needle biopsy, any method, any number of specimens



Deleted for 2015 are:



G0417 Surgical pathology, gross and microscopic examination, for prostate needle biopsy, any method, 21-41 specimens



G0418 Surgical pathology, gross and microscopic examination, for prostate needle biopsy, any method, 41-60 specimens



G0419 Surgical pathology, gross and microscopic examination, for prostate needle biopsy, any method, greater than 60 specimens



You cannot report 88305 for a prostate needle biopsy for a Medicare beneficiary. Code G0416 has a global, professional and technical component in the physician fee schedule and the appropriate modifier can be appended (TC or -26) depending on the billing circumstances. The national allowed charge for 2015 for code G0416 professional component will be $182.58 when compared to 88305 for 12 prostate biopsies will be $468.26.



As this is only a guideline for a Medicare beneficiary, continue to report 88305 for the appropriate number of separately identified specimens. APS Medical Billing is prepared to accommodate this change through a system program to convert 88305 to the appropriate “G” code if the patient is a Medicare or Medicaid beneficiary.

What's the purpose of the new Respiratory Specialty Team? Any thoughts? Is this a promotion or demotion? RST reps working with overlay reps in their territory and expected to grow Symbicort business? Is it "specialty" when customer calls are being counted and multiple reps in one office?

Everyone is wondering what'll happen to Pfizer. Who will we buy what drugs will i have next year and beyond.



Here's what I think may happen:



Part 1: Pfizer has separated into 3 operating divisions to establish a value for each division. GEP is the largest in terms of revenue as you know. The most common speculation is that Pfizer will sell the GEP and keep GIP and VOC under the Pfizer name. At that point Pfizers market cap will drop below 100b.

Part 2: it's no longer advantageous for Pfizer to buy a foreign company for the tax advantage. Especially considering any purchase will be at a serious premium.

Part 3: WE will be purchased by a foreign company. That company will grab up Pfizers cash horde, cut much of our sales force, and pay less taxes while doing it



There will be companies lining up to grab our cash. Especially after we shift dozens of generic product potential liability issues to the GEP buyer.
